Step 1: New Customer Profile

By submitting the above, you attest that the information entered is accurate. All information provided, including name and payment method, MUST match the Named Insured on the certificate of insurance (COI). If a DBA name is used you may be required to provide proof of ownership. Incomplete, inaccurate, or non-matching information may result in your rental being delayed or declined.
Step 2: Insurance
E-mail your Certificate of Insurance to rentals@lightscameragrip.com .
Insurance Requirements:
MEE LLC must be named as Additionally Insured/Loss Payee
Our address must be listed on the COI as follows:

Minimum Coverage Required:
- General Liability – Minimum $1,000,000
- Rented Equipment Coverage – The minimum dollar amount must meet or exceed the full replacement value of ALL of your rented equipment (including rentals from other companies)
- Hired Auto – mandatory if renting a vehicle
Step 3: Rental Contract & Credit Card Authorization
We will e-mail you our rental contract and a secure credit card authorization form once the above are complete.
