I, , as the cardholder, hereby authorize MEE LLC d/b/a Lights Camera Grip to charge and/or an authorization hold my credit card, without prior notice, for the following: Single or weekly rental charges, insurance deductibles, expendables and parts purchases, pay as you go expendables, damage to equipment, equipment cleaning and maintenance fees, lost or non-returned equipment, past-due equipment return, extended rental charges, returned checks, fuel, mileage, and any other charges covered in the signed Lease Agreement. Equipment not returned by the due date is considered lost or missing after 7 straight days, and could potentially accrue daily rental charges for the non-discounted rate of the item. Additionally, I understand that I am not limited to any specific dollar amount of authorization or charges; I am authorizing MEE LLC d/b/a Lights Camera Grip to charge me for any unpaid balance of any purchase or rental; I authorize MEE LLC d/b/a Lights Camera Grip to release equipment to my representative; The equipment does not need to be shipped to the billing address to be a valid credit card charge; I acknowledge that an electronic copy of this form and/or document shall constitute the same consent as a paper copy.
